We are excited you are considering enrolling your child with us!
The Eufaula Dormitory admission process includes several forms and an interview with our administrators.

- Forms: All forms are accessible from this page. You will be redirected to this page after you complete each form. You will also receive confirmation emails for your records that contain your answers and specific next-step directions. This will enable you to move easily through the process. Please keep in mind that each form must be completed in one sitting. You are not able to save and come back later.
- Interview: After all forms are complete, you will be scheduled for an interview with our administrators.
Please Note: Some forms require you to upload specific documents for your student. Please pause now to gather the following. You will not be able to submit your application without them.

- Birth Certificate (Form 1)
- CDIB or Tribal Citizenship Card (Form 1)
- Social security card (Form 1)
- Updated immunization record (Form 4)
- Soonercare card or private insurance card (Form 4)
- A recent photo of your student (Form 6)
- Additional documentation if applicable, such as court orders or other legal documents.

We recommend filling these forms out in order so that you can keep track of your progress. You will receive confirmation emails (sent to the email you enter in each form) so that you can confirm your submissions.

Extracurricular and Health Information
We are required to submit specific forms to state and Tribal agencies on behalf of your student to allow them to participate in extracurricular activities and to receive healthcare. Some of the information is in the form of PDFs that must be downloaded and completed by hand. You are able to submit the completed forms in several ways.

Paperwork Reduction Act Statement: This information is collected to identify each student’s instructional and residential program classification. It will be used to allocate appropriated funds on a weighted student unit formula. The information is supplied by the respondent to obtain or retain a benefit that is to provide appropriate schooling and the needed funding. It is estimated that this form will take an average of 15 minutes to complete. This includes the amount of time it takes to gather the information and fill out the form. If you wish to make comments on the form, please send them to Attn: Information Collection Clearance Officer Indian Affairs, 1849 C Street, N.W. MS-4141, Washington, DC 20240. The control number and expiration date are at the top right corner of the form. Please note that an agency may not conduct or sponsor, and a person is not required to respond to, a collection of information unless there is a valid OMB control number.
